Piles Treatment Without Surgery in Kanpur: Ksharsutra vs Conventional Methods

Piles (haemorrhoids) affect an estimated 75% of people at some point in their lives. In India, it is one of the most common anorectal conditions — yet many patients delay treatment due to fear of surgery, anaesthesia, and prolonged recovery. The good news: there is an effective Ayurvedic alternative that requires no general anaesthesia and has a recovery period of just 3–5 days.

What Are Piles?

Piles are swollen, inflamed veins in the rectum and anus. They are classified into four grades:

  • Grade I: Small haemorrhoids inside the anal canal. Bleed but do not prolapse.
  • Grade II: Larger haemorrhoids that prolapse during straining but return on their own.
  • Grade III: Prolapse during straining and must be manually pushed back.
  • Grade IV: Permanently prolapsed — cannot be pushed back.

Common causes include chronic constipation, low-fibre diet, prolonged sitting, pregnancy, and straining during bowel movements.

Conventional Treatment Options

Medications and lifestyle changes help with Grade I and early Grade II piles. Fibre supplements, stool softeners, sitz baths, and topical creams reduce symptoms but do not cure the underlying problem.

Rubber Band Ligation is an OPD procedure where a rubber band is tied around the base of the haemorrhoid to cut off its blood supply. It works for Grade II–III internal haemorrhoids but has a recurrence rate of 15–30% within 5 years.

Haemorrhoidectomy (surgical removal) is the conventional surgical approach. It is effective for Grade III–IV piles but requires general anaesthesia, a hospital stay of 1–2 days, and recovery of 2–4 weeks. Complications include pain, infection, and rarely, anal stenosis.

Ksharsutra: The Ayurvedic Alternative

Ksharsutra is an Ayurvedic para-surgical technique described in the ancient text Sushruta Samhita (600 BCE). The name comes from Sanskrit: Kshara (alkaline/caustic) and Sutra (thread). A linen thread is coated with alkaline preparations made from Snuhi latex (Euphorbia neriifolia), Haridra (turmeric) powder, and Apamarga Kshara (alkaline ash).

How Ksharsutra Treats Piles

  • The Ksharsutra thread is applied to the base of the haemorrhoid under local anaesthesia
  • The alkaline coating causes a controlled chemical cauterisation, cutting off blood supply
  • The pile mass gradually shrinks and separates over 5–7 days
  • The wound heals cleanly as the alkaline preparation also has antibacterial and anti-inflammatory properties
  • The entire procedure takes 20–30 minutes; patients are discharged within 5–6 hours
